Everything you recommends is done for a long time, so the behaviour you've just described seems to be impossible to achieve now.
I installed 4.6. And for the most part you are right. I uninstalled it and reinstalled it and the tools I didn't install didn't have listings in the menu. However, I still couldn't figure out how to get the right start menu items without doing it manually.
I did notice 1 thing that isn't updated, but that is really minor - the programs toolbar still has icons for all of the tools, even the ones that weren't installed.
